Filling out the online new-patient questionnaire before your first visit lets us spend your appointment on you, not paperwork.

Your evaluation
Your first visit is a comprehensive evaluation: we listen to your story, examine how your head, jaw, muscles, airway, and bite are functioning, and explain what we find in clear language.

Your plan
If treatment is appropriate, recommendations are individualized to your findings and explained before anything begins — every decision is made together.

Common Questions
Do I need a referral?
No referral is necessary for most patients. Referring physicians and dentists are always welcome to send records ahead of your visit.
Do you work with insurance?
To provide the highest level of care without the limitations of insurance, we are out of network with most insurance providers. However, we can provide a Superbill for you to submit to your insurance. Call or text us at (650) 369-9227 for more information.
Which forms should I fill out?
Please complete the online new-patient questionnaire before your first visit — there is one form for TMJ & orofacial pain and another for sleep apnea. Both are linked from our New Patient Forms page.
